Orthopedic Physical Therapist - Home Care Specialist

Position Overview

Active Life Physical Therapy Center is seeking a dedicated Orthopedic Physical Therapist to join our team in Keyport, New Jersey. This full-time, onsite role is designed for compassionate clinicians who are passionate about helping patients regain mobility and improve their quality of life through individualized treatment plans. The ideal candidate will have at least two years of experience and a Doctorate of Physical Therapy (DPT). You will work primarily in a home health care setting, providing therapeutic services to patients who require in-home rehabilitation following orthopedic surgeries or treatments.

Work Environment

This position requires you to work in the homes of patients, delivering personalized therapy in a comfortable and familiar setting for them. Key Responsibilities

• Conduct thorough patient evaluations including a comprehensive review of medical history and clinical assessments to establish the best course of treatment.
• Develop and communicate tailored treatment plans to patients and their families, ensuring they are actively involved in setting achievable rehabilitation goals.
• Implement treatment protocols with expertise, adapting them to meet the evolving needs of each patient based on real-time assessments of progress.
• Consult with other healthcare professionals when necessary to modify treatment plans and ensure that care is holistic and patient-centered.
• Document patient care activities promptly and accurately, including detailed evaluation notes, treatment progress summaries, and discharge plans to ensure compliance with regulations and improve care delivery.
• Manage schedules efficiently, ensuring all patient appointments are kept while striving to meet productivity expectations.
• Engage actively in departmental initiatives, including marketing efforts to promote our services and enhance community access to care.
• Lead and mentor junior staff and students, sharing your knowledge and experience while participating in continuing education activities.

Patient Population / Client Focus

Within this role, you will primarily serve individuals recovering from orthopedic procedures such as joint replacements, fractures, and sports-related injuries. Many of your patients may have limited mobility and require your expertise to regain their independence. Your compassionate approach and personalized care plan will play critical roles in their recovery journey.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited institution.
• Current state licensure as a Doctor of Physical Therapy (DPT).
• Minimum of two years of clinical experience in outpatient orthopedic settings.
• Current CPR certification, ensuring readiness to respond to emergencies.

Preferred Experience

Candidates with experience in home care services and familiarity with the latest physical therapy technologies and methodologies will be highly regarded. Knowledge in specialized treatment techniques such as manual therapy or post-operative rehabilitation protocols is a plus.

This position offers a competitive salary ranging from USD 100,000 to 120,000 annually, complemented by an excellent benefits package.
