Work Environment

This role is based onsite, as you will be traveling to patients' homes to provide therapy services. Key Responsibilities

• Deliver high-quality, patient-centered therapy by managing the entire therapeutic process for an assigned group of patients, coordinating care with physicians and other healthcare team members.
• Conduct thorough patient assessments to develop and implement individualized care plans, focusing on therapeutic goals and enhancing patient engagement.
• Administer physical therapy interventions, ensuring compliance with legal regulations and professional ethics while following the physician's plan of care.
• Evaluate patients' therapeutic, rehabilitative, and functional status to create tailored plans that reflect patient needs and desired outcomes.
• Provide skilled therapy services directly within the patient's home, employing specialized techniques aimed at improving their condition and functionality.
• Educate patients and their families on therapeutic exercises, techniques, and the use of any necessary medical devices or appliances for continued care outside of therapy sessions.
• Prioritize patient caseload management, assess staffing needs, and coordinate daily assignments to optimize workflow and efficiency.

Patient Population / Client Focus

Your role will involve working with a diverse patient population, including both adults and pediatric clients experiencing various conditions ranging from post-surgical recovery to chronic illness management. Qualifications

• A current and valid Physical Therapist license in the state of New Jersey.
• A Doctor of Physical Therapy (DPT) degree from an accredited institution.
• Basic Cardiac Life Support (BCLS) certification or the willingness to obtain it within 30 days of employment start date.
• No prior experience is required, making this an ideal position for new graduates eager to make a difference in the community.

Preferred Experience

• Experience in pediatric or orthopedic therapy, while not mandatory, is highly desirable and may enhance your effectiveness in this role.
• Previous experience in home care settings or with telehealth methods is an advantage as well.
