Full-Time Community-Focused Nurse Practitioner for Family Health

Work Environment

This position is a full-time onsite role based in Boulder City, Nevada, primarily working from our Home Care facility. You will be part of a collaborative team environment, which includes physicians, pharmacists, and nurses, all dedicated to improving patient care and community health. Your schedule will typically be from 9:00 a.m. to 6:00 p.m., with some flexibility for evening clinics until 8:00 p.m., and occasional weekend outreach events.

Key Responsibilities

• Conduct comprehensive health assessments of patients, utilizing advanced diagnostic skills to provide tailored care.
• Diagnose and manage both acute and chronic medical conditions, focusing on preventive care strategies that enhance health maintenance.
• Collaborate with community stakeholders to design and execute health education programs aimed at promoting wellness and disease prevention.
• Provide culturally competent care while advocating for patients’ rights to ensure everyone receives the healthcare they need without barriers.
• Coordinate referrals to specialists and ensure effective follow-up care to maintain continuity in patient health management.
• Maintain meticulous and compliant medical records as per regulatory requirements, utilizing electronic health record systems effectively.
• Engage with the community through health fairs and outreach initiatives to improve health literacy and accessibility of services.
• Stay informed on the latest community health trends, policy changes, and best practices in patient care.

Patient Population / Client Focus

This role serves a diverse population, particularly focusing on families within the Boulder City area. Many of our patients comprise vulnerable demographics with limited access to healthcare resources. By prioritizing individual and community needs, you will be instrumental in shaping the quality of care provided to those who may otherwise go underserved.

Qualifications

• A current Nurse Practitioner certification.
• Active nursing license in the state of Nevada.
• A Bachelor’s or Master’s degree in nursing from an accredited institution.
• A minimum of 3 years’ clinical experience, ideally in community health or public health settings.
• Demonstrated interpersonal and communication skills, with the ability to establish rapport with a variety of patient populations.
• A strong commitment to social justice and a passion for advocating for health equity among vulnerable communities.

Preferred Experience

• Experience in developing community health initiatives and participating in outreach activities.
• Familiarity with electronic health record (EHR) systems and other healthcare technologies.
• Leadership skills with the ability to mentor less experienced healthcare team members.

Compensation and Benefits

The salary for this position ranges from USD 120,000 to USD 130,000 annually, commensurate with experience and qualifications.
