Work Environment

The work setting for this position will be primarily onsite within our assisted living facility in Henderson, Nevada. You will have the opportunity to work with a diverse patient population, including older adults requiring various levels of medical support. The facility is designed to foster a warm and welcoming atmosphere, encouraging open communication and collaboration among staff and residents. This role requires flexibility, with scheduled hours from 9:00 a.m. to 6:00 p.m., including occasional evening clinics until 8:00 p.m. Participation in weekend community health events may also be necessary to engage with the broader population.

Key Responsibilities

• Conduct comprehensive health assessments and screenings, utilizing evidence-based practices to provide thorough evaluations of individual and family health statuses.
• Diagnose and manage chronic and acute medical conditions, developing patient-centered care plans that align with each person's health goals and needs.
• Engage with community-based organizations to design and implement educational programs that promote health literacy and empower patients to make informed health decisions.
• Advocate for patients' rights, ensuring they receive compassionate and culturally sensitive care while navigating healthcare services and resources.
• Coordinate referrals and follow-up care, collaborating with specialists and other healthcare providers to ensure seamless transitions in the patient care continuum.
• Maintain meticulous medical records in adherence to legal and regulatory requirements while ensuring the confidentiality and dignity of patients.

Patient Population / Client Focus

Qualifications

• Master's degree in Nursing from an accredited institution.
• Current Nurse Practitioner certification with an active nursing license in the state of Nevada.

Compensation and Benefits
