Work Environment

This full-time position requires your presence onsite at our facility in Henderson, where you will collaborate closely with a multidisciplinary team of healthcare providers and support staff. The typical work schedule will span from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach events or health fairs during weekends may also be necessary. We anticipate an average workweek totaling approximately 40 hours, providing a balanced work-life integration.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for individuals and families, including evaluations of physical, mental, and emotional health.
• Diagnose and manage both acute and chronic healthcare conditions while placing a strong emphasis on preventive care and health promotion.
• Collaborate with local community organizations and stakeholders to devise and execute health education programs that address community-specific health issues.
• Deliver culturally sensitive care, ensuring that advocacy for patients' rights and access to healthcare services is a priority.
• Coordinate referrals and follow-up care to ensure continuity and quality of care for all patients.
• Maintain detailed medical records in compliance with state and federal regulatory standards.
• Engage in community outreach initiatives, including health fairs, to provide vital health screenings and elevate health awareness within the community.

You will be working within an assisted living facility where the patient population primarily consists of elderly individuals requiring varying levels of medical and emotional support. Qualifications

• Possession of a current Nurse Practitioner certification along with an active state nursing license.
• A Master’s degree in nursing from an accredited program.
• A minimum of three years of clinical experience, particularly within community health or public health settings.

Compensation and Benefits
