Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to identify the emotional and developmental needs of children in hospice care.

• Participate in the development and implementation of Individualized Family Service Plans (IFSP), ensuring a tailored approach to each child's unique situation.

• Monitor measurable outcomes and provide feedback to families and colleagues as a vital member of the interdisciplinary team.

• Offer direct support to families by providing essential information and resources to help them navigate their child’s developmental challenges.

• Utilize evidence-based practices to design joint plans with families, focusing on enhancing the child's development through the Natural Learning Environment.

• Employ a coaching model to teach families effective techniques for engaging with their child in a nurturing manner.

• Adhere to federal and state regulations to maintain the highest standards of care and ensure the safety of all involved.

• Foster strong relationships with children, families, and team members to promote a collaborative and supportive education environment.

Patient Population / Client Focus\n

This role primarily focuses on young children, infants, and toddlers who require specialized support due to serious health conditions. Our services aim to facilitate their development during critical formative years, providing emotional and practical support to families addressing the complexities of hospice care. Qualifications\n

• A Master's Degree or Doctorate in Social Work is essential.

• Active licensure as an LCSW in Oregon and compliance with the Department of Public Health regulations.

Compensation and Benefits\n
