Key Responsibilities\n

• Conduct psychosocial assessments of individuals, families, and groups to determine their mental health needs and create effective treatment plans.

• Deliver graduate-level clinical social work or counseling interventions suited to each client, ensuring approaches are evidence-based and personally tailored.

• Perform initial comprehensive assessments on patients and their families, effectively triaging cases for the most appropriate interventions.

• Maintain clear and consistent communication with team members regarding assessment outcomes, treatment interventions, and updates within interdisciplinary care plans, through thorough documentation and regular meetings.

• Provide ongoing case management to assigned patients, coordinating with hospital staff and community professionals to ensure comprehensive, equitable care.
\n
• Adhere to ethical guidelines in timely completion of all necessary documentation, maintaining transparency and accountability within the department.

• Implement Cognitive Behavioral Therapy (CBT) alongside other clinical treatment modalities to support clients in achieving their individual service and recovery goals.

Patient Population / Client Focus\n

You will work closely with a diverse range of clients, including individuals and families experiencing various mental health challenges. Our outpatient services cater to adolescents and adults, providing essential support for issues such as anxiety, depression, trauma, and relationship difficulties. Qualifications\n

• Valid licensing as a Mental Health Counselor, including qualifications as either LMFT, LPC, or LCSW, in accordance with state regulations.
