Key Responsibilities\n

• Conduct comprehensive assessments to identify and address the mental health needs of clients, adapting treatment plans as necessary to ensure optimal outcomes.

• Maintain open and ongoing communication with supervisors, direct care staff, and relevant stakeholders regarding client progress, ensuring that everyone involved in the care process is informed.

• Develop and provide a schedule of appointments, home visits, and client monitoring to facilitate structured observation and accountability.

• Coordinate home visits and assessments, involving chart reviews and interviews to gather comprehensive data for effective treatment planning.

• Guarantee the effective execution of the treatment plan, ensuring adherence to ethical standards and best practices in counseling.

• Engage actively as a member of interdisciplinary team meetings to collaborate on client care and program improvement initiatives.

• Document monthly and quarterly progress notes accurately, tracking client development and program efficacy.

• Organize certification trainings for staff to ensure competency in program implementation and documentation practices.

• Provide ongoing follow-up through interviews and home visits, reinforcing the necessary support for clients’ ongoing and evolving needs.

• Act as a liaison with the broader mental health community, facilitating resource access and promoting developmental goals.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related mental health field is essential.

• An active and unrestricted license as a Professional Counselor (LPC) in the state of Arizona is required.
