Work Environment\n

This position allows for a remote work setting, providing flexibility to the LCSW while also enabling them to interact with patients and their families from the comfort of their homes. Regular team meetings will be held virtually to maintain effective communication and ensure aligned efforts in patient care. The work hours are structured from Monday to Friday, 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend shifts based on the scheduling needs of our clients.

Key Responsibilities\n

• Conduct thorough assessments of patients referred for mental health support and develop individualized treatment plans based on their unique needs.

• Provide psychotherapy services to hospice patients and their families, navigating complex emotional and psychological challenges during end-of-life care.

• Collaborate with an interdisciplinary team of healthcare professionals to ensure holistic care; participate in case reviews and treatment team meetings.

• Document all patient interactions, treatment plans, and progress notes accurately within our electronic health records system, maintaining compliance with HIPAA regulations.

• Educate patients and their caregivers about available resources and coping strategies to manage the psychosocial aspects of terminal illness and grief.

• Engage in continuous professional development and remain updated on best practices related to hospice care and mental health interventions.

Patient Population / Client Focus\n

The primary patient population includes individuals facing serious illness and end-of-life transitions, along with their families and caregivers. Our focus is on providing comfort and support, addressing the mental health needs that arise during this challenging time. The LCSW will approach each patient with empathy and respect for their diverse backgrounds, beliefs, and experiences.

Qualifications\n

• A master’s degree in Social Work from an accredited institution and a valid LCSW license in New Jersey.

• Expert knowledge in assessment, diagnosis, and treatment planning specifically geared toward patients with serious or terminal illnesses.

• Strong organizational and planning abilities to manage a diverse caseload efficiently.

Preferred Experience\n

Compensation and Benefits\n

We are pleased to offer a competitive salary range of $85,000 to $90,000 annually, along with a comprehensive benefits package.
