Work Environment\n

This full-time role provides the flexibility of working remotely, allowing you to conduct assessments and coordinate services from a location that suits your needs. However, occasional in-person visits to clients may be necessary to ensure effective care and support. Our work hours are generally Monday to Friday from 8:00 AM to 4:30 PM, with the expectation of some flexibility to accommodate varying client needs, including occasional evening or weekend hours and on-call availability.

Key Responsibilities\n

As a Licensed Clinical Social Worker at Bliss Reformation And Wellness Center, your responsibilities will include:

• Conducting thorough evaluations of the psychosocial needs of seniors, systematically considering emotional, mental, and social factors that impact their well-being.

• Collaborating with clients and their families to create personalized care plans that align with individual goals and preferences.

• Providing counseling and emotional support to help seniors and their families navigate life transitions, including bereavement and chronic illness challenges.

• Advocating for clients’ rights, ensuring they have access to necessary community resources and services.

• Working closely with interdisciplinary teams, including nurses, caregivers, and healthcare professionals, to coordinate comprehensive client care.

• Regularly monitoring clients' well-being through visits, assessing living conditions, and immediately addressing any safety or health concerns.

• Facilitating effective communication and conflict resolution among family members involved in client care.
\n
• Educating clients and families about available community resources, support groups, and programs to foster social engagement.

• Maintaining thorough and accurate documentation related to client assessments, care plans, progress notes, and relevant interactions in compliance with state regulations.

• Staying updated on best practices and policies in social work to ensure compliance with ethical standards.

This position centers specifically on seniors receiving hospice care, a sensitive and essential service aimed at enhancing their end-of-life experience. Qualifications\n

The ideal candidate will possess:

• A Master’s degree in Social Work from an accredited institution, along with a current, valid Colorado state license as a Licensed Clinical Social Worker (LCSW).

Preferred Experience\n
