Key Responsibilities\n

• Conduct thorough assessments and create comprehensive treatment plans tailored to individual client needs, encompassing crisis plans, risk assessments, and behavioral guidelines.

• Complete all necessary documentation promptly and ensure compliance with regulatory standards.
\n
• Utilize evidence-based screening tools effective as advised by the clinical team to facilitate accurate assessments.

• Collaborate actively with teams and programs to design and implement positive behavior support plans aimed at enhancing client engagement.

• Participate actively in licensing and accreditation processes to maintain the highest standards of care.
\n
• Serve as a liaison to various service systems, state agencies, and community partners to facilitate client access to resources.

• Maintain open communication to meet contract standards and further the mission of Health Link Now within the community.
\n
• Develop strategic partnerships with local, state, federal, and private organizations to advance our programs and identify new funding opportunities.
\n
• Participate in ongoing professional development through conferences, training, and representation in statewide meetings related to mental health care.
\n
• Uphold and promote the agency’s mission, values, philosophy, and principles in every aspect of your work.

• Perform additional duties as assigned by the Clinic Director to support operational needs.

Patient Population / Client Focus\n

You will be working with a diverse patient population that includes individuals seeking support for mental health issues, substance use recovery, developmental disabilities, and neurological conditions. The role requires a tailored, person-centered approach to meet the emotional and practical needs of each client effectively.

Qualifications\n

• A Master’s Degree in the mental health field is required.

• Possession of an Independent LMHC License.
