This position is fully remote, allowing you to provide care from the comfort of your home while maintaining a work-life balance. Our telehealth facility utilizes a variety of digital platforms to ensure seamless communication with patients and collaboration with team members. Your work typically follows a Monday to Friday schedule from 8:00 AM to 5:00 PM, with occasional evening shifts based on patient requirements. Key Responsibilities\n

• Conduct comprehensive mental health assessments, carefully analyzing how medical conditions impact patients' psychological well-being.

• Facilitate both individual and group therapy sessions, tailoring interventions to fit the unique circumstances of each patient, such as those managing chronic pain alongside depression or anxiety.

• Collaborate with physicians, nurses, and other healthcare providers to ensure coordinated and effective treatment plans that enhance patient outcomes.

• Provide urgent crisis intervention and emotional support in mental health emergencies to help stabilize patients during critical moments.

• Educate patients on understanding the intricate connection between their mental health and medical conditions, empowering them with strategies to manage their health better.

Patient Population / Client Focus\n

You will be working with a diverse range of patients, particularly those grappling with co-occurring mental health and medical issues. This includes clients suffering from chronic illnesses who may experience compounded mental health challenges, as well as individuals facing the complexities of substance use disorders. Your role will be crucial in understanding these patients holistically and providing integrated therapeutic support that addresses all dimensions of their health.

Qualifications\n

• A Master’s or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution is required.

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW), Licensed Clinical Professional Counselor (LCPC), or Licensed Marriage and Family Therapist (LMFT) in the state of Illinois.

• Proven knowledge and experience addressing the mental health needs of individuals with co-occurring medical conditions.

• Experience in delivering integrated care through collaboration with healthcare professionals in a clinical setting.

• Exceptional interpersonal and communication skills, enabling meaningful engagement with patients and colleagues.

• Ability to maintain professional boundaries and adhere to ethical guidelines while ensuring patient confidentiality.

• Familiarity with electronic health record systems and relevant computer applications to document and manage patient care effectively.

Preferred Experience\n

Compensation and Benefits\n
