Fountain Hills Fitness Center is actively seeking a dedicated Full-Time Orthopedic Physical Therapist to join our team, with a primary focus on delivering exceptional care in a home care setting in Port Charlotte, Florida. Work Environment

This position is fully onsite, providing therapeutic services within the patients' homes. You will work Monday to Friday, completing a 40-hour workweek that emphasizes a balance of professional development and personal well-being. Key Responsibilities

The Orthopedic Physical Therapist will:

• Conduct comprehensive evaluations of patients referred by physicians, assessing their physical condition and functional abilities.
• Design and implement customized treatment plans based on patient assessments, focusing specifically on orthopedic conditions, while also having exposure to pediatric, neurologic, and sports medicine populations.
• Utilize a range of therapeutic techniques to improve patients' conditions, following the established plan of care and adjusting as necessary based on ongoing assessments.
• Demonstrate outstanding customer service, ensuring all patient interactions reflect our commitment to excellence and compassionate care.
• Educate patients and their caregivers on treatment procedures, promoting understanding and involvement in the rehabilitation process.
• Work collaboratively with a multidisciplinary team, engaging in regular meetings to discuss patient progress and enhance overall care strategies.
• Meet or exceed defined utilization and productivity goals to contribute to the organization's overall success.

Patient Population / Client Focus

Our client focus extends across a diverse range of populations, including individuals facing orthopedic injuries, rehabilitation needs from sports-related conditions, and general practical physical therapy. Qualifications

The ideal candidate must possess:

• A Bachelor's or Doctorate in Physical Therapy from an accredited program (APTA or CAPTE approved).
• A valid state license in Physical Therapy to practice in Florida.
• Basic Life Support (BLS) certification from the American Heart Association.
• Strong communication skills, both verbal and written, facilitating effective interaction with patients, caregivers, and the healthcare team.
• Competence in using word processing and computer programs for documentation and communication purposes.
• The ability to cultivate strong relationships with patients to motivate them towards achieving their functional goals.

Preferred Experience

While not mandatory, the following experiences will enhance your application:

• Previous experience in a home care setting or outpatient orthopedic practice.
• Exposure to diverse patient populations and familiarity with specialized therapeutic techniques.
• A proven track record of building and maintaining community relationships to enhance patient care.
