Work Environment

This position is based in a home care setting, requiring an on-site presence to deliver therapy directly to patients in their residences. You will work Monday to Friday, collaborating with a diverse healthcare team to ensure the highest standards of patient care.

Key Responsibilities

Your primary duties as a Physical Therapist will include:

• Developing and implementing tailored physical therapy treatments to prevent disability, alleviate pain, and enhance muscle function based on individual care plans.
• Supervising Physical Therapy Assistants to ensure adherence to the established plans of care while providing guidance and mentorship.
• Creating and conducting diagnostic evaluations of patients' functional abilities to accurately assess needs and progress.
• Evaluating home environments to recommend modifications that enhance patient independence and facilitate effective rehabilitation.
• Providing home-based physical therapy services as prescribed, including administering therapeutic exercises, traction, and massages.
• Assessing and adjusting prosthetic and orthotic devices, including recommendations for necessary modifications.
• Contributing to the development, review, and update of care plans ensuring alignment with best practice methodologies.
• Thoroughly maintaining clinical records and documentation to ensure compliance and quality of care.
• Participating in interdisciplinary team case conferences to enhance treatment planning and patient evaluations.
• Educating clients and families on device usage and therapeutic exercises to empower them throughout the rehabilitation process.
• Conducting functional assessments and resource evaluations for community support.

Compensation and Benefits
