This position is remotely-based, providing flexibility while ensuring that our dedicated LCSW can effectively engage with clients from the comfort of their homes or designated remote work settings. Working with a team of health professionals, the LCSW will collaborate closely to provide comprehensive care and address the emotional and psychological needs of clients in hospice care.

Key Responsibilities\n

• Conduct empathetic and thorough assessments of clients who exhibit symptoms of major mental illnesses and severe emotional disorders.

• Develop and implement individualized treatment plans that cater to each client’s unique needs and circumstances, ensuring access to necessary services.

• Provide brief, focused prevention, treatment, and recovery services, utilizing evidence-based practices to enhance client outcomes.

• Maintain a caseload that includes assessment, individual therapy, family therapy, and group therapy, adapting therapeutic approaches to suit diverse client populations.

• Actively participate in team meetings and case conferences to discuss client progress and share insights for improved service delivery.

• Prepare and submit documentation and reports accurately and timely, in compliance with organizational standards and regulations.

• Assist clients in crisis by employing appropriate intervention techniques, ensuring their safety and well-being.

• Provide on-call coverage as required, offering support during urgent circumstances that may arise after hours.
\n
• Engage in ongoing professional development and training to enhance skills and stay current on best practices in social work and mental health care.

Patient Population / Client Focus\n

The LCSW will work primarily with clients in a hospice setting, who are often facing significant end-of-life challenges. This role requires a deep understanding of the unique emotional and psychological needs of patients, as well as their families during this critical time. Sensitivity to grief, loss, and bereavement is essential, as is the ability to facilitate discussions around difficult topics with empathy and professionalism.

• A Master's degree in Social Work and a valid LCSW license in Connecticut is required.

Compensation and Benefits\n
