This position will be based in a fully remote work setting, allowing you to deliver therapy sessions from the comfort of your own home. Key Responsibilities\n

• Provide expert clinical consultation to clinical staff regarding mental health issues across diverse patient populations, including geriatric clients and those with substance use disorders.

• Effectively interpret and communicate pertinent information about patients and their families, including critical social histories, to ensure comprehensive care coordination.
\n
• Lead the planning and facilitation of decision-making processes concerning individualized patient care plans, particularly with consideration of the home environment and external resources.
\n
• Develop and review treatment plans based on established standards and in collaboration with other providers to optimize client outcomes.
\n
• Attend and contribute to regular team meetings, envisioning innovative approaches to enhance patient therapies and intervention strategies.
\n
• Complete detailed progress notes following client sessions efficiently and accurately to maintain high-quality documentation and ensure continuity of care.

• Engage in continued education and training initiatives to stay current with mental health trends and procedures.
\n
• Perform any additional duties as needed to support the team and the organization's goals.

Patient Population / Client Focus\n

In this role, you will work primarily with a diverse clientele, including individuals facing mental health challenges, substance use disorders, and the unique needs of geriatric patients. You will assess their mental health concerns and develop tailored therapeutic strategies that empower them to achieve better mental health and personal growth.

Qualifications\n

• A Master’s Degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling from an accredited institution is required.

• Possession of an active and independent license to practice, such as LMHC, LCSW, or LMFT, is essential.
\n
• Proven ability to document clinical interactions and progress notes in a clear and efficient manner following each client session.

Preferred Experience\n

• A minimum of two years of experience working in a clinical environment, particularly with diverse clinical populations.

• Demonstrated experience navigating electronic health record (EHR) systems to maintain accurate patient documentation.

• Experience in consultation and collaboration with interdisciplinary teams is highly desirable.
