Key Responsibilities

• Conduct thorough and comprehensive assessments to diagnose both acute and chronic medical conditions in patients during home visits.
• Develop and implement individualized, evidence-based treatment plans in partnership with interdisciplinary teams comprising physicians, nurses, and specialists.
• Order and interpret diagnostic tests such as lab work and imaging studies to inform medical decisions and adjust treatment plans accordingly.
• Prescribe appropriate medications, therapies, and lifestyle modifications, ensuring optimal patient engagement and adherence to treatment regimens.
• Educate patients and their families on managing diseases, understanding medication regimens, and preparing for post-discharge care.
• Participate actively in weekly case discussions and interdisciplinary rounds to continuously improve patient care strategies.
• Maintain accurate and up-to-date electronic medical records (EMRs), documenting patient encounters, treatment plans, and changes in condition effectively.
• Engage in quality improvement initiatives, contributing innovative ideas for enhancing patient care protocols and ensuring compliance with industry standards.
• Stay abreast of the latest healthcare advancements, regulatory changes, and best practices to uphold the highest level of care.

Qualifications

Compensation and Benefits

The compensation for this position ranges from USD 120,000 to 220,000 annually, commensurate with your experience and qualifications.
