Key Responsibilities\n

• Conduct in-depth psychosocial assessments to evaluate clients’ mental health needs, identifying specific strengths and challenges in a hospice context.

• Utilize evidence-based therapies such as cognitive-behavioral therapy, mindfulness practices, and family systems therapy to address issues of grief, depression, and anxiety.

• Collaborate with the healthcare team to develop, implement, and monitor comprehensive treatment plans that are responsive to patients’ evolving needs.

• Manage crisis intervention procedures by performing risk assessments, developing safety plans, and acting swiftly to support clients in distress.

• Advocate for clients’ rights and needs while connecting them with essential community resources, including support groups and bereavement services.

Compensation and Benefits\n

Community Health Connection offers a competitive salary range of USD 70,000 to 100,000 per year, based on experience and qualifications.
