Key Responsibilities\n

As a Hospice Care Specialist, your day-to-day responsibilities will include:

• Conducting comprehensive assessments of patients’ mental, emotional, and social needs related to their end-of-life transition.

• Developing and implementing personalized treatment plans using evidence-based practices tailored to each patient's situation.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

• A current and valid license to practice as a Licensed Clinical Social Worker (LCSW) in California.
