The work hours are Monday to Friday from 9:00 AM to 5:00 PM, providing a structured schedule to support your professional responsibilities without compromising personal time.

Key Responsibilities\n

As part of your role as a Licensed Clinical Social Worker, you will be responsible for the following key tasks:

• Acting as a vital liaison between clients, families, and outpatient providers, ensuring that everyone is informed and engaged in the care process.

• Interacting with insurance companies to secure pre-certifications and manage utilization effectively.
\n
• Participating in comprehensive discharge and aftercare planning to facilitate smooth transitions for clients leaving our care.

• Providing therapeutic meal support while modeling a healthy relationship with food, an essential aspect of overall well-being.

• Delivering safe, empathetic, and affirming support tailored to each client's unique background and intersecting identities.

Patient Population / Client Focus\n

Your focus will primarily be on older adults who are facing end-of-life challenges. This demographic requires specialized attention to their unique emotional and psychological needs, and you will tailor your approach to provide comfort and dignity during this critical time. You will also engage with their families, offering support and guidance as they navigate the complexities of hospice care.

Qualifications\n

To be considered for this role, candidates should possess the following qualifications:

Compensation and Benefits\n

We offer a competitive salary range of $90,000 to $105,000 annually, commensurate with experience and qualifications. In addition to competitive pay, our benefits package includes:
