Work Environment\n

This position is 100% remote, enabling you to deliver professional support while enjoying the flexibility of working from home. You will work Monday through Friday, from 9:00 AM to 5:00 PM, ensuring ample time to focus on your clients and professional development.

Key Responsibilities\n

• Act as a critical liaison between families, outpatient providers, and hospice staff to facilitate open communication and support during a patient’s care journey.

• Engage with insurance companies to navigate pre-certification and utilization management, ensuring that all necessary approvals are secured for patient services.

• Participate actively in discharge planning and aftercare coordination, providing comprehensive support to transitioning patients and their families.

• Offer therapeutic meal support and model healthy relationships with food, creating a supportive atmosphere for clients that emphasizes dignity and respect.

• Deliver safe, empathetic, and affirming care tailored to each client's unique circumstances, while actively considering their intersecting identities and backgrounds.
