Key Responsibilities\n

• Conduct thorough mental health assessments for patients and their families, considering their unique backgrounds, emotional states, and specific circumstances related to terminal illness.

• Develop personalized treatment plans that address the mental health concerns of clients while promoting emotional resilience and coping strategies.

• Utilize evidence-based therapeutic approaches including cognitive-behavioral therapy (CBT), grief counseling, and bereavement support to assist patients and families in managing their emotional challenges.

• Facilitate individual and family therapy sessions, helping clients navigate complex feelings associated with terminal illness and loss.
\n
• Provide crisis intervention and support in high-stress situations, with the ability to de-escalate emotional distress and guide families to appropriate resources.

• Collaborate with an interdisciplinary team, including physicians, nurses, and chaplains, to ensure a coordinated and holistic approach to each patient's care.

• Offer psychoeducation to families about mental health issues related to grief and loss, empowering them with strategies to manage their emotional well-being.
\n
• Maintain accurate documentation of assessments, treatment plans, and patient progress in compliance with legal and ethical standards, utilizing electronic health record (EHR) systems.

• Engage in ongoing professional development, attending staff meetings and training sessions to enhance your skills and keep abreast of current best practices.

Patient Population / Client Focus\n

In this role, you will work closely with patients who are experiencing terminal illnesses and their families, addressing a range of emotional responses including anxiety, depression, and fear. Your expertise will support patients in processing their experiences and finding peace in their journey while providing strategies for their loved ones to cope with the impending loss.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• A current and valid LCSW licensure in California.

• At least two years of post-licensure experience, ideally in a mental health or hospice setting where empathy and understanding are critical.

• Strong knowledge of mental health disorders, effective treatment modalities, and trauma-informed care principles.

• Proven skills in assessing mental health needs and developing tailored treatment approaches.

• Demonstrated experience in providing both individual and family therapy, with strong crisis management abilities.

• Exceptional communication and interpersonal skills, particularly in establishing trust and rapport with diverse populations.

• Experience utilizing EHR systems, with a high degree of computer literacy.

Compensation and Benefits\n
