Key Responsibilities\n

Your primary responsibilities will include:

• Conducting comprehensive evaluations and assessments to determine individual family needs and child development status.

• Participating actively in the development and implementation of Individualized Family Service Plans (IFSP), ensuring that the strategies align with best practices.
\n
• Monitoring outcomes collaboratively with your team, facilitating regular reviews to adjust services based on evolving family priorities and child's developmental progress.
\n
• Delivering evidence-based early intervention support and individualized strategies to families, enhancing their capacity to foster their child's development.
\n
• Building rapport with children and their families to establish a trusting and supportive environment.
\n
• Utilizing a coaching model and natural environment teaching techniques to facilitate learning in everyday contexts.
\n
• Ensuring compliance with all federal and state regulations to maintain the highest standards of care and safety for all clients.

Patient Population / Client Focus\n

In this role, you will focus on supporting families with young children facing disabilities or delays in development, particularly in a hospice context. Your work will be pivotal in guiding these families through difficult times, providing emotional support, practical strategies, and resources to navigate their challenges while celebrating their child's unique journey.

Qualifications\n

The ideal candidate will possess the following qualifications:

• A Master’s Degree or Doctorate in Social Work.

• Licensure by the Department of Public Health – LCSW is required.
\n
• A valid driver’s license for any required community visits.
\n
• Strong organizational skills and the ability to effectively manage multiple cases and schedules.

• Excellent communication skills, fostering positive relationships with families and interdisciplinary team members.

• A commitment to adapting interventions to meet diverse learning styles and needs of children and families.

Preferred Experience\n

Compensation and Benefits\n
