Key Responsibilities\n

• Serve as a liaison between clients, their families, and outpatient service providers to ensure a comprehensive care plan that aligns with their needs.

• Engage with insurance companies to facilitate pre-certification processes and manage utilization effectively.

• Participate in discharge and aftercare planning, providing critical support and guidelines for continuity of care.

• Deliver therapeutic meal support that models healthy eating behaviors and contributes to overall client wellness.

• Provide safe, empathetic, and affirming care tailored to the diverse identities of each client, ensuring a respectful and supportive therapeutic relationship.

Qualifications\n

Compensation and Benefits\n
