Key Responsibilities\n

• Conduct thorough psychosocial assessments to gain insight into patients' emotional, social, and financial needs, thereby forming a foundation for customized care plans.

• Join forces with medical professionals to develop comprehensive treatment strategies that alleviate the psychosocial barriers patients face during detox.

• Provide individual and group counseling sessions focused on areas such as grief processing, adjustment challenges, and coping strategies tailored to the recovery process.

• Coordinate with community organizations to facilitate access to post-discharge services, ensuring that patients have continuity of care and support after leaving our facility.

• Deliver emotional support and educational resources to patients and families regarding treatment options, coping mechanisms, and local resources available to them.

• Participate actively in multidisciplinary meetings, providing insights and recommendations pertaining to social work and patient care.

• Offer crisis intervention services and immediate support during critical situations, ensuring patients and families feel safe and heard.

• Develop and implement strategic discharge plans, connecting patients with community resources suited to their ongoing needs.

• Maintain thorough documentation in adherence to hospital policies, regulatory standards, and best practices for confidentiality and accuracy.

You will primarily work with adults facing substance use disorders, including those beginning their recovery process in a detox setting. Qualifications\n

• Master's degree in Social Work from an accredited program.

Compensation and Benefits\n
