Work Environment

The role is located onsite in our assisted living facility, where you will collaborate closely with an interdisciplinary team, including doctors, nurses, and support staff. This setting allows you to foster meaningful relationships with patients and their families in a warm and supportive environment. Our facility is designed to accommodate the unique needs of our residents, providing comfort and security while promoting independence.

Key Responsibilities

• Conduct comprehensive health assessments for residents, diagnosing and treating a variety of complex medical conditions.
• Administer therapeutic interventions such as splinting, suturing, wound care, and other minor procedures, ensuring a high standard of care.
• Utilize critical thinking skills and adhere to evidence-based practice standards to optimize patient treatment plans.
• Document all patient encounters accurately and thoroughly, ensuring compliance with regulatory standards and facilitating seamless communication among caregivers.
• Analyze diagnostic test results, interpret data, and make informed decisions regarding follow-up care and treatment strategies.
• Provide education to patients and their families regarding acute illness management, preventive care, and overall health maintenance.
• Engage in sensitive discussions about end-of-life care options, addressing patients' and families' needs with empathy and clarity.
• Maintain effective communication with patients, families, medical power of attorney, primary care providers, and other relevant stakeholders in the care process.
• Identify and proactively resolve issues while adhering to clinical protocols and safety standards.
• Demonstrate compassion and cultural competence in all interactions, fostering trust and rapport.
• Support and enhance partnerships with our Direct Healthcare Management Team (DHMT) as well as remote teams, participating in regular clinical meetings and training sessions.
• Serve as a professional ambassador for Highland Medical Care Hospital, upholding a standard of care that reflects our core values.
• Exhibit leadership in practice, consistently prioritizing patient well-being and advocating for their care.

Patient Population / Client Focus

The Nurse Practitioner will primarily care for residents of the assisted living facility, which includes a diverse population with varying healthcare needs. Many residents may have chronic illnesses, cognitive impairments, or require assistance with daily living activities. Thus, the ability to provide both acute and long-term care with sensitivity and understanding is essential in fostering a supportive living environment.

Qualifications

• A current and active nursing license in the state of Texas is mandatory.
• A Bachelor’s degree in nursing from an accredited institution is required; a Master's degree is preferred.
• At least three years of clinical experience in a nursing role, particularly in geriatric care, acute care, or similar environments.
• Demonstrated strong analytical and problem-solving skills to navigate complex clinical scenarios.
• Exceptional communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
• Ability to work independently and take initiative while remaining a collaborative team player.

Preferred Experience

• Experience in an assisted living facility or related setting is highly desirable.
• Familiarity with technology-driven healthcare solutions and digital patient care management is a plus.
• Background in coordinating care for complex patients with interdisciplinary teams.

Compensation and Benefits

We offer a competitive salary in the range of USD 110,000 to 130,000 annually , commensurate with qualifications and experience. Our comprehensive benefits package includes:
