As a Nurse Practitioner, you will work closely with an interdisciplinary team, including physicians, nurses, and other allied health professionals, ensuring comprehensive and coordinated care for our residents. This role emphasizes direct patient interaction, collaborative decision-making, and continuous quality improvement to enhance patient outcomes.

Key Responsibilities

• Conduct thorough assessments and accurate diagnoses for patients experiencing acute and chronic pain conditions.
• Design and implement personalized treatment plans, including the use of medication management, physical rehabilitation strategies, and other therapeutic interventions.
• Order and interpret diagnostic tests such as X-rays and lab results to inform clinical decisions.
• Provide patient and family education on pain management strategies, promoting health literacy and self-management techniques.
• Collaborate with fellow healthcare team members in regular case discussions and care coordination meetings to deliver integrated care solutions.
• Document all patient interactions and updates in our electronic health record (EHR) system, maintaining compliance with regulatory standards.
• Engage in quality improvement initiatives aimed at enhancing clinical practices and patient satisfaction outcomes.
• Stay current with advancements in pain management methodologies and healthcare regulations to provide the best care possible.

Patient Population / Client Focus

You will primarily work with elderly residents in an Assisted Living Facility who may be experiencing complex pain management issues related to chronic conditions, including arthritis, neuropathy, and post-surgical recovery. Understanding the unique challenges and emotional aspects faced by elderly patients in managing pain effectively is crucial to this role.

Qualifications

• Current certification as a Nurse Practitioner with an active Texas state nursing license.
• A Bachelor's degree in nursing, with a preference for candidates holding additional advanced degrees or certifications in pain management.
• A minimum of three years of clinical experience in pain management or a related specialty.
• Exceptional communication and interpersonal skills to effectively engage patients and collaborate with team members.
• Strong analytical, critical thinking, and problem-solving abilities to navigate complex patient scenarios.

Preferred Experience

Experience in an assisted living or long-term care setting, particularly with chronic pain populations, is highly desirable. Proficiency in electronic health record (EHR) systems and experience in quality improvement processes would also be advantageous.

Compensation and Benefits

We offer a competitive salary range of $110,000 to $130,000 annually, commensurate with skills and experience.
