Work Environment

This position is based onsite at an Assisted Living Facility in Houston, Texas, where you will work collaboratively with healthcare teams to ensure that residents receive the highest standard of care. Your work schedule will typically be Monday through Friday from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Additionally, participation in community outreach events and health fairs during weekends may be required. Your average workweek will total approximately 40 hours, providing a structured yet impactful work routine.

Key Responsibilities

• Conduct comprehensive health assessments for individuals and families, gathering detailed medical histories and performing necessary screenings to identify health concerns.
• Diagnose and manage both acute and chronic medical conditions, offering preventive care services aimed at health promotion and disease prevention.
• Collaborate with community partners to develop and implement effective health education programs that address local health needs.
• Provide culturally sensitive care, ensuring an understanding of and respect for diverse backgrounds and advocating for patients’ rights to access healthcare services.
• Facilitate referrals and coordinate follow-up care to maintain continuity in patients’ healthcare journeys.
• Maintain accurate and up-to-date medical records compliant with regulatory standards, ensuring proper documentation of all patient interactions and care plans.
• Engage in community outreach initiatives, attending health fairs and local events to offer health screenings, raise awareness, and educate community members.
• Stay informed on current public health trends, evidence-based practices, and relevant healthcare policies impacting patient care.

Patient Population / Client Focus

You will work directly with a diverse population, including elderly residents of the assisted living facility, many of whom may have complex health needs. Your role will involve addressing not only their physical health but also their emotional and social well-being, ensuring that each resident receives comprehensive, compassionate care tailored to their unique circumstances.

Qualifications

• Current certification as a Nurse Practitioner (NP) and an active nursing license in the state of Texas.
• A Master’s degree in Nursing from an accredited program, providing you with a robust foundation for clinical practice.
• Minimum of 3-5 years of clinical experience, preferably in community health, family medicine, or public health settings.

Preferred Experience

• Experience working within assisted living or similar environments is highly desirable.
• Previous involvement in community health initiatives and an understanding of public health strategies.

Compensation and Benefits
