Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate the social, emotional, and environmental factors affecting patients and their families, focusing intently on their specific illnesses and life circumstances.

• Develop, implement, and adjust individualized treatment plans that incorporate appropriate social work interventions, aimed at meeting the ongoing needs of patients pre and post hospitalization.

• Work collaboratively within a multidisciplinary healthcare team, ensuring that all relevant parties, including patients and families, are actively involved in the planning and delivery of care.

• Facilitate effective discharge planning, including coordination with consulting physicians and third-party payers to ensure a smooth transition from hospital to home or other care settings.

• Provide education and counseling to patients and families regarding coping strategies related to illness and situational adaptations, supporting mental health and emotional adjustment.

• Utilize crisis intervention techniques as needed, offering individual, group, and family therapy tailored to the patient’s specific requirements and circumstances.

• Serve as a valuable resource for staff members during cases involving complex psychosocial needs, ensuring that everyone is well-informed and equipped to handle various challenges.

• Present educational workshops or conferences, showcasing expertise in psychosocial oncology practices and promoting community awareness regarding mental health issues.

• Conduct clinical evaluations for oncology patients related to mental health concerns such as depression and anxiety, providing short-term counseling as necessary.

• Participate in on-call rotations to manage after-hours support requests and emergencies, ensuring continuous care for our patients.

• Engage in community service efforts that strengthen service accessibility for cancer survivors and foster engagement with local community resources and support programs.

Patient Population / Client Focus\n

The focus of this role will predominantly be on patients receiving hospice and palliative care, including oncology patients navigating the complexities of terminal illnesses. Qualifications\n

• A Bachelor’s Degree in Social Work with a valid LCSW license in the state of Oregon is required.
