As part of a hospice center, you will be part of an interdisciplinary team focused on compassionate, client-centered care. You will have access to the resources and tools necessary to perform assessments, develop treatment plans, and coordinate care with external stakeholders seamlessly.

Key Responsibilities\n

Your role as an LCSW will involve a range of critical responsibilities, including:

• Conducting thorough assessments to determine the counseling needs of clients, ensuring that individual circumstances are understood and addressed.

• Utilizing the DSM (Diagnostic and Statistical Manual) criteria to accurately diagnose co-occurring substance use disorders and aligning treatment plans with ASAM (American Society of Addiction Medicine) guidelines.

• Co-developing personalized treatment plans that incorporate evidence-based practices, ensuring that counseling strategies effectively meet the diverse needs of clients.
\n
• Facilitating individual, family, and group therapy sessions that promote healing, resilience, and a path toward long-term recovery.
\n
• Documenting all interactions, treatment sessions, and assessments meticulously in client records to ensure compliance and continuity of care.

• Managing your caseload efficiently while participating in ongoing treatment planning and clinical charting as required.

• Coordinating with family members and referral sources, including legal and educational institutions, to provide comprehensive support throughout the recovery process.

• Addressing crisis situations by implementing necessary interventions, drawing upon a collaborative multi-disciplinary team approach.

Patient Population / Client Focus\n

In this role, you will work with a diverse range of clients, including those experiencing co-occurring mental health disorders and substance use issues. Many clients may have complex life histories involving trauma, loss, and significant psychological barriers to recovery. By employing trauma-informed care principles in your practice, you create a safe and supportive environment that empowers clients on their pathways to recovery.

Qualifications\n

To succeed in this role, candidates must possess the following qualifications:

• A valid LCSW license to practice in the state of Oregon.

• A Bachelor’s Degree in Social Work or a related field; higher education is preferred.

Compensation and Benefits\n
