Key Responsibilities\n

• Conduct thorough assessments of client counseling needs, utilizing comprehensive evaluation techniques to determine appropriate interventions.

• Perform substance abuse assessments based on DSM (Diagnostic and Statistical Manual) criteria to ascertain the necessary level of care using ASAM (American Society of Addiction Medicine) guidelines.

• Prepare and document the substance abuse counseling segment of each client's treatment plan, ensuring it aligns with best practices and organizational policies.

• Develop individualized discharge and continuum of care plans that facilitate a smooth transition to appropriate levels of support, contributing to ongoing recovery efforts.

• Maintain meticulous records of all counseling services provided, including documentation of assessments, referrals, and follow-ups in each client's clinical record.

• Manage a caseload of clients, including coordinating case management activities and ensuring clinical charting is completed in a timely manner.
