Key Responsibilities\n

• Utilize electronic medical records (EMR) to diligently monitor patient progress, document treatment initiatives, and communicate effectively regarding care plans.

• Collaborate closely with an interdisciplinary team, anticipating patient needs and adjusting care plans as necessary to ensure holistic support.

• Provide vital support and education to patients and families regarding transition plans and available resources to facilitate smoother transitions.

• Maintain up-to-date knowledge of government and private payer networks to aid in appropriate transitions and navigate healthcare logistics.

• Engage with community resources to coordinate comprehensive care tailored to the unique needs of each patient.
\n
• Advocate for the rights of patients and their families, ensuring access to supportive services and protective resources as mandated by law.

• Participate proactively in care system processes aimed at preventing readmissions by identifying at-risk patients and intervening early.
\n
• Conduct transition conferences and utilize technology tools to address potential readmission risks effectively.
\n
• Ensure thorough clinical handoffs for patients identified as at risk, coordinating referrals and follow-ups.

• Provide counseling and support to address financial or social needs that may impact a patient’s treatment and recovery.

• Other duties as assigned to fulfill the holistic care model.

Compensation and Benefits\n
