Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for clients and their families, identifying strengths, needs, and emotional concerns related to end-of-life care.

• Provide tailored individual, family, and group therapy sessions, employing various evidence-based therapeutic modalities, including cognitive-behavioral therapy and motivational interviewing.

• Develop personalized treatment plans that include goals, actionable interventions, and ongoing progress assessments, ensuring they align with the unique cultural needs of each client.

• Engage collaboratively with a multidisciplinary team to coordinate robust, comprehensive care providing seamless transitions between various service providers.

• Implement crisis intervention strategies by evaluating risk factors and formulating safety plans to protect vulnerable clients.

• Advocate for clients’ rights socially and legally while connecting them with necessary community resources to enhance care accessibility.

• Participate actively in community outreach initiatives aimed at raising awareness around hospice care and mental health, fostering community engagement and support.

Qualifications\n

• A master's degree in Social Work from an accredited institution is required for this position.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) is essential.

Compensation and Benefits\n
