This role is based in the beautiful educational landscape of Punta Gorda, Florida, where you will work onsite in a home care setting. Key Responsibilities

• Conduct comprehensive evaluations of patients' physical conditions and functional capabilities in their home environments.
• Develop and implement individualized treatment plans that incorporate various therapeutic interventions, including exercise prescriptions, manual therapies, and the use of equipment to manage pain and enhance mobility.
• Monitor patient progress and REVise treatment goals as necessary to ensure optimal outcomes are achieved.
• Document therapeutic interventions and outcomes meticulously, maintaining accurate patient records that reflect treatment plans and progress.
• Supply patients with education regarding their conditions and engage them in their own care, empowering them to take an active role in their rehabilitation journey.
• Guide and train patients, caregivers, and families on the usage of assistive devices and adaptions for maximizing mobility and self-care.
• Continuously enhance your professional skills and knowledge through participation in workshops, online courses, and professional societies.

Patient Population / Client Focus

As a Home Care Physical Therapist with Quantrics, you will primarily serve children and adults recovering from a diverse range of conditions, including post-surgical rehabilitation, neurological disorders, and chronic pain conditions. You will work with patients in their own homes, providing personalized care while adapting to each individual's unique environment. Your role will be central to helping patients regain their independence and improve their daily functioning, with a holistic approach to managing their rehabilitation needs.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Current valid state license in Physical Therapy.
• Strong written and verbal communication skills to effectively interact with patients and their families.
• Proficiency in computer skills, including basic word processing and electronic documentation systems.
• Flexibility in scheduling and adaptability to varying patient needs and environments.

Preferred Experience

• Two years of previous clinical experience as a Physical Therapist is preferred but not mandatory.
• A willingness to learn and grow within a collaborative, patient-centric environment.

Compensation and Benefits
