Regular business hours will be observed, although some evening and weekend hours may be required based on patient needs and caseload assignments.

Key Responsibilities\n

• Collaborate with the interdisciplinary rehabilitation team to develop individualized treatment plans that account for the psychosocial dimensions of patients' health and recovery goals.

• Guide patients in navigating community resources such as support groups, vocational rehabilitation programs, and housing assistance, ensuring they have access to necessary support services.

• Deliver educational sessions and workshops to patients and their families regarding available community resources, enhancing their understanding of post-rehabilitation options.

• Participate actively in discharge planning, ensuring a smooth transition for patients back into their communities and daily lives.

• Document patient progress and interventions within our electronic health record system, ensuring accurate and up-to-date information for the clinical team.

Patient Population / Client Focus\n

Our organization principally serves a diverse patient demographic, including individuals recovering from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. The LCSW will focus on addressing the specific emotional and psychosocial needs of this patient population, helping them to overcome barriers to recovery and reintegration into society.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Current and valid LCSW licensure in the state of California.
