Work Environment

This role is primarily performed onsite within clients' homes across Jersey City, allowing you to bring therapy directly to those needing it most. The work week is structured from Monday to Friday, promoting a healthy work-life balance.

Key Responsibilities

As a Physical Therapist with our organization, you will be responsible for:

• Conducting thorough diagnostic evaluations of clients' functional abilities, tailoring therapy plans to individual needs.
• Administering direct physical therapy treatment aimed at enhancing mobility, alleviating pain, and preventing disability, while adhering strictly to established care plans.
• Supervising Physical Therapy Assistants to ensure compliance with therapy protocols and effective care delivery.
• Evaluating client home environments and recommending necessary modifications to facilitate rehabilitation and independence.
• Providing therapeutic exercises and specialized interventions, such as manual therapy techniques and functional activities.
• Administering treatments such as traction and rubdowns, promoting recovery and improving client comfort.
• Collaborating in the development, review, and update of individualized care plans in interdisciplinary team settings.
• Maintaining meticulous clinical records and documentation, submitting them as required, to ensure continuity of care.
• Educating clients and their families on the safe use of therapeutic equipment and strategies for at-home rehabilitation.
• Conducting functional assessments and evaluating community resources to support clients' rehabilitation journeys.

Patient Population / Client Focus

Your work will engage with a diverse patient population, primarily consisting of individuals managing chronic pain, recovering from surgery, or dealing with mobility impairments. Each client's unique circumstances necessitate a customized therapeutic approach, ensuring therapy is effective in enhancing their quality of life. You will play a critical role in empowering clients to regain their independence and improve functional outcomes through direct, individualized care.

Qualifications

The ideal candidate will possess:

• A Bachelor's degree from an accredited physical therapy program.
• Active Physical Therapy licensure in New Jersey.
• At least one year of clinical experience in a home health or acute care setting.
• Current CPR certification and a valid driver's license with automobile insurance.
• Excellent interpersonal and communication skills, with a demonstrated ability to collaborate effectively with multidisciplinary teams.
• A strong commitment to clinical excellence and a passion for patient care.
• Familiarity with relevant state and federal healthcare regulations.

Preferred Experience

While not essential, the following attributes will enhance your application:

Compensation and Benefits
