Key Responsibilities\n

• Conduct thorough, ongoing assessments to identify the unique mental health needs of each patient.

• Maintain open lines of communication with supervisors, healthcare team members, and the patient’s support network, including family members and guardians, to discuss progress and adjust treatment plans as needed.

• Develop and provide a tentative schedule to supervisors that outlines planned appointments, home visits, and collateral observations.
\n
• Coordinate and conduct home visits to follow up with patients, ensuring comprehensive support meets the requirements of in-home therapy.

• Implement and monitor the effectiveness of individualized care plans, ensuring that treatment objectives are met with fidelity.

• Engage actively in Interdisciplinary Team Meetings to collaborate with other professionals and contribute to holistic patient care strategies.

• Complete detailed monthly and quarterly progress reports reflecting patient progress and necessary adjustments to treatment plans.
\n
• Organize and lead training sessions to ensure that staff and team members are competent in the implementation of program methodologies and necessary documentation protocols.

• Provide ongoing support through follow-up interviews and observations, facilitating continuous care that adapts to evolving patient needs.

• Maintain precise and comprehensive records that are easily accessible for review by authorized individuals, ensuring adherence to confidentiality and ethical standards.

• Act as a liaison with the community and mental health stakeholders to enhance the development and delivery of mental health services.

• Contribute to the creation of procedures and training materials aimed at improving existing programs for client support.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Possession of active and unrestricted licensure as a Professional Counselor (LPC) within the state of Arizona.

• Minimum of two years of relevant counseling experience, preferably in a clinical setting.

• Demonstrated knowledge and practice in family and couples therapy, trauma-informed care, or substance use counseling is highly desirable.

• Strong understanding of legal and ethical guidelines relevant to mental health practice.
