Key Responsibilities\n

• Conduct thorough psychosocial assessments to better understand the mental health concerns, strengths, and resources of patients and their families facing end-of-life issues.

• Develop and execute individualized treatment plans that address the unique needs and preferences of each patient, integrating evidenced-based therapeutic interventions.

• Facilitate individual and family therapy sessions focused on coping strategies, grief, and bereavement, using modalities such as narrative therapy and cognitive-behavioral interventions.

• Provide crisis intervention and emotional support for patients and families during times of need, offering a compassionate presence during critical emotional moments.

• Collaborate with the healthcare team, including physicians, nurses, and chaplains, to create a comprehensive and coordinated care plan that holistically addresses the needs of patients.

• Lead support groups and psychoeducation workshops for patients and families to strengthen coping tools and community support networks.

• Maintain up-to-date and detailed documentation of sessions and assessments, adhering to ethical and legal guidelines while ensuring patient confidentiality.

As a Licensed Clinical Social Worker in our hospice center, you will be working closely with patients facing terminal illnesses and their families. Qualifications\n

Compensation and Benefits\n
