True Living Wellness Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our dynamic team in Dallas, Texas. Work Environment

This is a fully remote position, allowing flexibility while working in collaboration with our interdisciplinary team. Though based out of Dallas, Texas, our goal remains focused on delivering optimal patient care regardless of location. Regular business hours apply, with some flexibility to accommodate patient and team needs.

Key Responsibilities

• Conduct comprehensive psychosocial assessments through in-depth interviews, observations, and thorough medical record reviews to ascertain emotional and social factors impacting patient recovery.
• Deliver individualized counseling and support to patients and their families to address emotional challenges and adjustments during the rehabilitation process.
• Collaborate with rehabilitation staff—such as therapists and physicians—to devise personalized treatment plans that incorporate patients' psychosocial needs alongside their medical requirements.
• Facilitate access to community support resources, including support groups, vocational rehabilitation services, and housing assistance, that enhance recovery outcomes.
• Educate patients and families about available community resources and post-rehabilitation options, ensuring they are well-prepared for life beyond the care center.
• Actively participate in discharge planning efforts to ensure seamless transitions to home or other care settings, promoting continued healing and support.

Patient Population / Client Focus

You will work with a diverse patient population facing a range of rehabilitation challenges. Your role is crucial in helping patients cope with adjustments—be it after significant medical events or chronic illnesses—enabling them to navigate both emotional and logistical pathways towards reintegration into their communities.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Valid and active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong communication skills with a capacity to engage sensitively with patients and their families in various states of emotional distress.
• Proficient in conducting psychosocial assessments and developing tailored support strategies.

Preferred Experience

• Previous experience in a rehabilitation or medical setting is highly desirable.
• Working knowledge of issues surrounding grief, coping mechanisms, and adjustment disorders.
• Ability to work collaboratively within a multidisciplinary team is essential.

Compensation and Benefits

The salary range for this position is competitive, from USD 75,000 to 100,000 annually, reflecting your skills and experience, along with a sign-on bonus.
