This position is performed remotely, leveraging telehealth technologies to connect with patients from the comfort of their homes. You will enjoy a flexible work schedule from Monday to Friday, typically from 9:00 AM to 5:00 PM, with occasional evening shifts to accommodate specific patient needs. Key Responsibilities\n

• Conduct thorough mental health assessments that account for each patient's unique medical history, cognitive abilities, and social circumstances.

• Devise and implement personalized treatment plans that incorporate evidence-based interventions, adjusted for the specific needs of the elderly, such as cognitive behavioral therapy, supportive counseling, and family dynamics.

• Provide high-quality therapeutic interventions and crisis management to address urgent mental health issues, ensuring that patients receive immediate support.

• Work with multidisciplinary teams, including geriatric psychiatrists, primary care providers, and social workers, to orchestrate integrated patient care and improve overall health outcomes.

• Educate patients and their families about effective management strategies for mental health conditions, treatment pathways, and valuable community resources available to them.

• Maintain meticulous documentation and records that comply with legal, ethical, and best practice standards, utilizing electronic health record (EHR) systems efficiently.

• Engage actively in regular team meetings and professional development sessions to stay updated on best practices and emerging trends in geriatric mental health.

Patient Population / Client Focus\n

This role focuses primarily on the geriatric population, comprising individuals typically aged 65 and older. You will encounter a diverse range of mental health needs, including but not limited to those associated with depression, anxiety, grief, adjustment disorders, and cognitive impairments. Understanding the life experiences of older adults and their emotional responses is crucial in this specialized area of mental health.

Qualifications\n

• A Master’s or doctoral degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.

• Current licensure as an LCSW, LPC, or LMFT is mandatory in the state of Oregon.

• Minimum of two years of post-licensure counseling experience is preferred, with a focus on geriatric populations.

• Extensive knowledge of geriatric mental health disorders and effective treatment modalities.

• Proficient in individual and family therapy techniques tailored to older adults, demonstrating empathy and understanding.

Compensation and Benefits\n
