Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of senior clients, evaluating emotional, mental, and social factors that impact their well-being.

• Develop and implement personalized care plans in collaboration with clients and their families, aligning services with their unique preferences and goals.

• Provide counseling and emotional support, assisting seniors and their families as they encounter life transitions, such as end-of-life issues, grief, and illness.

• Advocate for clients’ access to community resources and services, ensuring they receive the support necessary for improved quality of life.

• Coordinate with interdisciplinary teams, including nurses, caregivers, and other healthcare professionals, to create a seamless network of care.

• Perform regular monitoring visits to evaluate clients' well-being, address safety concerns, and assess their living conditions.

• Facilitate effective communication among family members involved in care decisions, mediating any conflicts that may arise.

• Educate clients and their families about available community resources and support services, fostering social engagement and improving overall wellness.

• Document client interactions, assessments, care plans, and progress notes accurately in accordance with organizational standards.

• Stay current on social work practices, policies, and regulations to ensure compliance with ethical standards.

Patient Population / Client Focus\n

Your primary focus will be on seniors in a hospice setting, dealing with complex health and life challenges. This requires a deep understanding of aging-related issues, including chronic illness management, mental health concerns, and the social dynamics of caregiving. You will support clients through critical life stages, providing both emotional and practical resources tailored to their needs.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW).

• At least two years of experience in a social work role, preferably within a community center or geriatric care setting.

Compensation and Benefits\n

The compensation for this full-time position ranges from $60,000 to $100,000 per year, commensurate with experience and qualifications.
