Key Responsibilities\n

As a Licensed Clinical Social Worker, you will:

• Conduct culturally sensitive and age-appropriate comprehensive biopsychosocial assessments to identify mental health, substance use, and medical needs, ensuring holistic care in a primary care setting.

• Deliver tailored behavioral health services, including counseling, intake assessments, treatment planning, crisis interventions, and referrals to relevant substance abuse treatment programs.

• Document and maintain accurate electronic health records using our secure systems, including progress notes, evaluations, and safety assessments to track client progress and outcomes.

• Serve as a liaison between clients and community resources, providing social work support, facilitating access to medication assistance programs, and connecting families with vital local services.

• Collaborate with Quality Improvement teams to innovate and implement data collection processes aimed at measuring service effectiveness and client satisfaction.

Patient Population / Client Focus\n

Your primary focus will be on children and their families facing a variety of challenges, including emotional and behavioral issues, family dynamics, and social determinants of health. Qualifications\n

The successful candidate will possess:

Compensation and Benefits\n
