Work Environment

You will be working onsite as part of an interdisciplinary team in the patients' homes. Key Responsibilities

• Direct and implement individualized physical therapy treatment plans to help patients recover from injury, relieve pain, increase mobility, and restore function.
• Supervise Physical Therapy Assistants to ensure they deliver quality care aligned with the established treatment plan.
• Conduct assessments of patient capabilities and provide recommendations for home modifications that promote independence in daily activities.
• Perform therapeutic exercises, manual therapies, and other modalities as prescribed, ensuring patient safety and comfort at all times.
• Regularly evaluate and adjust treatment plans based on patients' responses and progress, communicating findings with their physician and care team.
• Document all clinical records accurately, including evaluations, care plans, progress notes, and discharge summaries, adhering to regulatory standards.
• Engage with patients and their families to educate them about therapies, self-care techniques, and appropriate equipment usage.
• Participate in interdisciplinary team meetings, sharing insights and feedback to enhance the overall treatment strategy.
• Remain accessible to support and guide Physical Therapy Assistants during their sessions with clients.

Patient Population / Client Focus

As a Physical Therapist at Confluent Rehabilitation, you will primarily work with a diverse patient population, including seniors recovering from surgeries, individuals with chronic conditions, and those experiencing post-acute rehabilitation needs. Your goal will be to empower these patients to achieve their rehabilitation goals while enhancing their quality of life within the community.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• Must possess a valid PT license in good standing in the state of New Jersey.
• Current CPR certification is a must-have for emergency readiness.
• A valid driver's license and automobile insurance to travel to patients' homes.
• Strong communication skills, with the ability to engage effectively with patients, families, and interdisciplinary teams.

Preferred Experience

We prefer candidates with at least two years of experience in physical therapy, particularly in home care or acute care settings. Familiarity with regulatory compliance and local healthcare policies is also beneficial.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 100,000 to 120,000, complemented by a robust benefits package.
