Work Environment\n

This position offers the flexibility of remote work, allowing you to conduct assessments and manage client caseloads efficiently from home. You will interact with clients primarily via telehealth platforms, ensuring that you can provide consistent care that adapts to their needs. The work schedule is Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours required to meet client needs. You may also need to be on-call to address urgent situations outside of standard working hours.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify the emotional, mental, and social needs of seniors.

• Develop and implement individualized care plans in collaboration with seniors and their families, integrating their specific goals and preferences.

• Provide counseling and emotional support to assist seniors and their families in navigating transitions, including health crises or lifestyle changes.

• Advocate for clients, ensuring they have access to essential community resources and services that enhance their quality of life.

• Collaborate with a multidisciplinary team, including healthcare professionals and caregivers, to deliver holistic care for seniors.

• Perform regular virtual visits to monitor clients’ well-being, assess living conditions, and address any safety concerns or changes in health status.

• Facilitate effective communication and mediate conflicts among family members to enhance the caregiving environment.

• Educate clients and families about local resources and support groups, promoting social engagement and overall well-being.

• Maintain detailed and accurate documentation of client interactions, assessments, and care plans, adhering to confidentiality and compliance requirements.

The primary focus of this role is on the senior population, particularly those facing challenges related to aging. Our clients may experience various psychosocial issues such as isolation, mental health concerns, or difficulties in accessing healthcare. Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

Compensation and Benefits\n
