Work Environment\n

This position offers the flexibility of a remote work setting, allowing you to provide care and support to clients in hospice from the comfort of your home while occasionally visiting clients as needed. Your typical workweek will span Monday to Friday, from 8:00 AM to 4:30 PM, although some flexibility is required to accommodate evening or weekend visits, ensuring that our clients receive timely support. You will also have on-call responsibilities to address urgent situations that may arise outside of regular hours.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, encompassing emotional, mental, and social domains to formulate tailored care strategies.

• Develop and implement individualized care plans in collaboration with clients and their families, ensuring alignment with their values and preferences.

• Provide empathetic counseling and emotional support to clients and their families, helping them navigate complex life transitions and emotional challenges.

• Advocate for clients’ needs and rights, ensuring they have access to necessary community resources and services, including healthcare, social support, and educational programs.

• Collaborate with interdisciplinary teams of healthcare professionals, such as nurses and caregivers, to enhance the overall quality of care provided to clients.

• Regularly visit clients to assess their well-being, monitor living conditions, and promptly address any emerging concerns related to safety and care.

• Facilitate communication and addressing conflicts among family members to promote collaborative care and understanding.

• Educate clients and families on available community resources, support groups, and programs that can enhance their social engagement and overall quality of life.

• Maintain thorough and accurate documentation of client evaluations, care plans, and interactions, using electronic health record systems to ensure compliance and continuity of care.

• Stay current with developments in social work practices and regulations to uphold ethical and professional standards in all aspects of service delivery.

The primary focus of this role is on homebound seniors receiving hospice care. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current licensure as a Licensed Master Social Worker (LMSW) in the state of Florida.

• A minimum of two years of relevant experience in social work, particularly in a hospice, geriatric, or community service environment.

• Demonstrated skills in assessment, counseling, and conflict resolution, with a client-centered approach.

Compensation and Benefits\n
