Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and their families to identify emotional, social, and environmental factors that impact their diagnosis and treatment.

• Formulate, develop, and implement individualized care plans utilizing evidence-based social work interventions tailored to each patient’s needs.

• Collaborate with the healthcare team, actively involving patients and families in discussions and decision-making processes regarding their treatment plans.

• Facilitate discharge planning across a multidisciplinary care continuum, coordinating with consulting physicians, third-party payers, and other healthcare providers to ensure appropriate post-discharge resources and support.

• Provide education and counseling to patients and families on issues related to coping with illness and transitioning through various stages of health care.
\n
• Engage in crisis intervention and deliver brief and long-term therapeutic options, including individual, group, and family therapies as needed.

• Act as a consultant and resource for other clinical staff, offering guidance on psychosocial issues impacting patient care.
\n
• Clinically assess patients for symptoms of depression and anxiety, providing short-term counseling as appropriate.
\n
• Develop educational materials and conduct presentations both for professional staff and at community health events, sharing knowledge on psychosocial oncology practices.

• Participate in the after-hours on-call rotation to support patient needs and respond to urgent psychosocial inquiries.
\n
• Contribute to the community through educational outreach, service consultation, and volunteering initiatives focused on the well-being of cancer survivors.

• Support the social work profession by mentoring new staff, promoting student training in oncology social work, and engaging in clinical research to elevate standards within the field.

Patient Population / Client Focus\n

The LCSW will primarily work with hospice patients, many of whom face terminal illnesses and complex psychosocial challenges. This role requires sensitivity and an understanding of the unique psychological and emotional needs of patients and families navigating end-of-life issues, making it essential to provide empathetic support and tailored interventions.

Qualifications\n

• A Bachelor’s Degree in Social Work (BSW) required; Master’s Degree in Social Work (MSW) is preferred.

• Active and unrestricted Licensed Clinical Social Worker (LCSW) certification in the State of Florida.

• Strong clinical assessment skills and ability to formulate care plans that address diverse patient needs.

• Excellent communication skills, with the ability to build rapport and effectively collaborate with a multidisciplinary team.

• Demonstrated experience working with patients in hospice or similar healthcare settings is a plus.

Compensation and Benefits\n

The salary for this role ranges from USD 70,000 to 100,000 per year, commensurate with experience and qualifications.
