Work Environment\n

This role offers the flexibility of remote work, allowing you to effectively support patients from the comfort of your home while interacting primarily through telehealth platforms. You will engage with a robust electronic health record (EHR) system to document care plans and patient encounters, ensuring compliance and coordination with the interdisciplinary healthcare team. Your work hours will be from Monday to Friday, 9:00 AM to 5:00 PM, providing a structured yet flexible work schedule.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate the emotional, social, and environmental factors affecting patients in a hospice care setting.

• Provide insightful counseling services, disease management strategies, and targeted health education tailored to meet the unique needs of each patient.

• Collaborate closely with patients and an interdisciplinary healthcare team to develop and implement customized care plans, focusing on behavior modification, coping skills, and quality-of-life improvements.

Qualifications\n

• A Master's degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Pennsylvania.

Compensation and Benefits\n

This role offers a competitive salary ranging from USD 90,000 to 100,000 per year, commensurate with experience, along with an attractive bonus structure.
