Work Environment

This position is primarily onsite, where you will provide physical therapy services directly within the clients' homes, fostering a supportive and conducive environment for rehabilitation. Working within a collaborative interdisciplinary team, you will directly influence patient outcomes and ensure a seamless integration of services that promote independence and quality of life in the comfort of familiar surroundings.

Key Responsibilities

• Develop and implement individualized treatment plans focused on improving patient mobility, functional capacity, and pain relief, aligned with the physician's directives.
• Conduct comprehensive evaluations to assess the client's physical abilities and environmental needs, ensuring effective recommendations for home adjustments to enhance independence.
• Oversee the work of Physical Therapy Assistants, providing guidance and support to ensure adherence to prescribed care plans while fostering their professional development.
• Engage in manual therapy techniques, therapeutic exercises, and the administration of physical agents, tailoring approaches to the patients' specific conditions and responses.
• Document patient progress through detailed clinical records, including initial evaluations, treatment plans, progress notes, and discharge summaries, ensuring compliance with regulatory standards.
• Participate in case conferences with an interdisciplinary team to discuss patient care strategies, making collaborative decisions to optimize treatment outcomes.
• Continuously monitor and adjust treatment strategies based on patient responses, providing feedback to the Case Manager and the client's physician.
• Educate patients and their families on the management of conditions, including the correct use of physical therapy modalities and devices to reinforce treatment efficacy.
• Maintain timely and accurate documentation for clinical and administrative purposes as per company policies, including billing and payroll documentation.
• Contribute to quality improvement initiatives aimed at enhancing patient experience and operational efficiencies.

Patient Population / Client Focus

The role centers on providing comprehensive rehabilitation services for a diverse population, including individuals recovering from surgery, managing chronic pain, or requiring assistance due to disability. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required; a Doctorate in Physical Therapy (DPT) is preferred.
• A minimum of one year of experience in a clinical setting, with a strong emphasis on direct patient care in home health or acute care environments.
• Current state licensure as a Physical Therapist in good standing, along with a current CPR certification.
• A deep understanding of regulatory standards and guidelines governing physical therapy practice in the homecare environment.
• Exceptional communication skills, with an ability to connect with patients and collaborate effectively with multidisciplinary teams.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 100,000 to 120,000, commensurate with experience and qualifications.
