Key Responsibilities\n

• Conduct thorough assessments of clients’ needs during telephone or video consultations, gathering relevant information to inform care plans.

• Provide crisis intervention to clients in distress, utilizing effective de-escalation techniques and immediate support strategies.

• Develop and implement individualized treatment plans aimed at achieving positive outcomes for clients and their families.

• Facilitate referrals to relevant social services, which may include housing, substance abuse treatment, and mental health resources.

• Maintain accurate and confidential client records in accordance with organizational policies and applicable regulations.

• Document interactions and prepare reports to monitor progress, assess the effectiveness of interventions, and improve service delivery.

• Participate actively in training sessions and supervision to enhance skills and receive professional guidance.

• Remain vigilant and sensitive to signs of child abuse or neglect, ensuring that necessary interventions are initiated.

Qualifications\n

• A Master’s degree in Social Work or a related behavioral health field is required.

• Current Connecticut license as a Clinical Social Worker (LCSW) is mandatory.
