Key Responsibilities\n

• Conduct comprehensive assessments of individuals’ mental health needs, ensuring tailored treatment plans are developed and modified as necessary.

• Maintain effective communication with various stakeholders, including supervisors, direct care staff, family members, and other relevant parties to discuss patient progress and care adjustments.

• Provide a detailed schedule of patient appointments, home visits, and follow-up observations, ensuring timely and effective continuity of care.
\n
• Coordinate and execute home visits for assessments and therapeutic interventions, providing an opportunity for in-depth evaluation of clients’ home environments.
\n
• Participate proactively in Interdisciplinary Team Meetings to contribute insights, share best practices, and collaborate on patient care strategies.
\n
• Document patient progress through thorough monthly and quarterly reports, providing measurable outcomes and continuity of care records.
\n
• Organize and deliver training sessions for staff on program plans and documentation practices, ensuring all team members are equipped with the necessary competencies for effective patient care.

• Perform follow-up interviews and home visits to provide ongoing support, ensuring that clients' evolving needs are met through adaptive therapeutic practices.

Patient Population / Client Focus\n

Our counselors work with a diverse client population, including both adults and youth, addressing a wide range of mental health issues such as depression, anxiety, trauma, and substance misuse. Qualifications\n

• A Master's or Doctorate degree in a relevant mental health field is required.

• Possession of active, unrestricted licensure as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW) in California.

• A minimum of two years of counseling experience in a clinical environment is required, demonstrating a solid foundation in therapeutic practices.
